What happened in Other:
During the operation (22.05.24) an observation card was written that there was not a proper platform to stand on while working to even out the cuttings. A rake was used to even out the cuttings.

What Was Planned

Initially we evaluated the need for risk assessments and follow up. The need for a cutting handling risk assessment was evaluated. The current operator and rig had already evaluated and tested the cutting handling set up so it was decided in cooperation with the rig that an additional risk assessment was not needed.

What Was Learned

I learned not to trust that previous operator have carried out the required risk assessments.

What Actions Are Required

Always review the cutting handling set up with focus on:
Is there a platform to stand on
Do we have the proper equipment to even out the cuttings in the skip
Is there a need for work rotation
Can the personnel be exposed to oil vapor
Any other risks?

What Was Done

This will be communicated to the HSEQ department by mail and the project risk checklist is updated.
